This disclaimer explains the nature and limits of the telehealth services offered through Project Preservation. Please read it carefully before using our services.
Nature of Telehealth
Project Preservation provides telehealth services through its licensed providers. Care is delivered remotely using secure electronic communication rather than an in-person visit. Telehealth has the same purpose as traditional care but relies on technology, and certain evaluations or treatments may not be appropriate to deliver remotely.
Not for Emergencies
Our services are not for medical emergencies. If you are experiencing a medical emergency, call 911 or go to the nearest emergency room. Do not use this platform to seek urgent or emergency care.
Not a Substitute for Primary Care
Our services are intended to complement, not replace, your relationship with a primary care provider. We do not act as your primary care provider, and you should maintain regular care with your own physician. You are encouraged to share information about your Project Preservation treatment with your other providers.
Provider–Patient Relationship
A provider–patient relationship is established only after a licensed provider reviews your information, determines that treatment is appropriate, and accepts you for care. Submitting an intake does not by itself create a treatment relationship or guarantee a prescription.
Where We Operate
Services are available to patients in the states where our providers hold applicable licensure, in accordance with state telehealth regulations. Availability of specific services may vary by state.
No Guarantee of Outcomes
Individual results vary. No specific outcome is promised or guaranteed. Treatment recommendations reflect a provider's clinical judgment based on the information you provide, and the accuracy and completeness of that information is your responsibility.
Technology Limitations
Telehealth depends on technology that may occasionally fail or be interrupted. If a technical issue affects your care, contact our concierge team so we can assist or arrange a follow-up.
