Project Preservation is committed to protecting the privacy of your health information. We are required by law to maintain the privacy of your protected health information (PHI), to give you this notice of our legal duties and privacy practices, and to follow the terms of the notice currently in effect.

How we may use and disclose your health information

Treatment

We use and share your PHI to provide, coordinate, and manage your care — including consultations with licensed providers, clinical review of your intake, and coordination with the compounding pharmacy that dispenses your medication.

Payment

We use and share your PHI to bill and collect payment for the services and products you receive.

Health care operations

We use and share your PHI to run our practice, improve your care, and contact you about your treatment, refills, and services.

Other uses and disclosures

We may use or disclose your PHI without your authorization when required or permitted by law, including for public health activities, to comply with legal or regulatory requirements, and to prevent a serious threat to health or safety.

Uses that require your written authorization

Most uses and disclosures for marketing purposes, the sale of your PHI, and other uses not described in this notice will be made only with your written authorization. You may revoke that authorization, in writing, at any time.

Your rights

• Get an electronic or paper copy of your health record
• Ask us to correct or amend your health record
• Request confidential communications
• Ask us to limit what we use or share
• Get a list of those with whom we've shared your information
• Get a copy of this privacy notice
• Be notified in the event of a breach of your unsecured PHI

Our responsibilities

We are required by law to maintain the privacy and security of your PHI and to notify you promptly if a breach occurs that may have compromised the privacy or security of your information. We will not use or share your information other than as described here unless you tell us we can in writing.

Changes to this notice

We may change the terms of this notice, and the changes will apply to all information we have about you. The new notice will be available upon request and posted on our website.

Complaints

If you believe your privacy rights have been violated, you may file a complaint with us using the contact below, or with the U.S. Department of Health and Human Services, Office for Civil Rights. You will not be penalized for filing a complaint.
