PROJECT PRESERVATION TELEHEALTH SERVICES
Effective Date: May 12, 2026
1. Overview
This Membership Agreement (“Agreement”) is entered into between you (“Member,” “you,” or “your”) and Project Preservation (“PP,” “we,” “us,” or “our”), a telehealth platform offering peptide therapy, longevity consultations, and related wellness services. By enrolling in a Project Preservation membership, you agree to be bound by the terms set forth in this Agreement.
Project Preservation is a wellness and longevity platform that facilitates access to licensed healthcare providers and compounded peptide therapies through a concierge telehealth model. We are not a medical practice. Medical services are rendered by independently licensed healthcare providers (“Providers”) who are contracted through our platform.
2. Membership Enrollment
Membership is available to individuals who are at least 18 years of age and reside in a state where our Providers are licensed to practice. Enrollment requires the completion of a health intake form, identity verification, and payment of applicable fees. By enrolling, you represent that the information you provide is accurate, truthful, and complete.
Project Preservation reserves the right to decline or revoke membership at any time, for any reason, including but not limited to: ineligibility based on health history, geographic restrictions, failure to comply with Provider recommendations, or misrepresentation of personal or medical information.
3. Scope of Services
Your membership provides access to the following services, subject to medical eligibility as determined by your assigned Provider:
- Initial telehealth consultation with a licensed healthcare provider
- Personalized peptide protocol recommendations
- Ongoing follow-up consultations at scheduled intervals
- Prescription management and coordination with our licensed compounding pharmacy partner
- Secure messaging and concierge support via our designated communication channels
Services do not include emergency medical care, primary care services, specialist referrals, diagnostic imaging, laboratory work (unless otherwise specified), or any services outside the scope of peptide therapy and longevity wellness.
4. Medical Decision-Making
All clinical decisions, including the prescribing of peptide therapies, are made solely by your licensed Provider based on their independent medical judgment. Project Preservation does not practice medicine, diagnose conditions, or prescribe medications. We serve as a platform connecting Members with Providers and facilitating access to compounded therapies.
You understand that your Provider may determine, at any time, that peptide therapy is not appropriate for you based on your health history, contraindications, lab results, or other clinical considerations. In such cases, your membership may be modified or terminated, and applicable refund terms will apply.
5. Fees and Payment
Membership fees are outlined at the time of enrollment and may include a one-time consultation fee, recurring membership fees, and the cost of prescribed peptide therapies, which are billed separately through our pharmacy partner. All fees are non-refundable except as expressly stated in this Agreement.
You authorize Project Preservation to charge the payment method on file for all applicable fees. Failure to maintain a valid payment method may result in suspension or termination of your membership.
Peptide therapy costs are separate from and in addition to your membership fee. Pricing for peptide protocols will be communicated prior to fulfillment, and your authorization is required before any prescription is sent to the pharmacy.
6. Cancellation and Refund Policy
You may cancel your membership at any time by contacting our concierge team via the designated communication channel. Cancellation will take effect at the end of your current billing cycle. No partial refunds will be issued for unused portions of a billing period.
If you have not yet received a consultation or any clinical services, you may request a full refund of your membership fee within fourteen (14) days of enrollment. Once a consultation has been completed, the consultation fee is non-refundable. Peptide orders that have been compounded and shipped are non-refundable.
Peptide orders that have not yet been compounded may be eligible for cancellation at the discretion of the pharmacy
7. Member Responsibilities
As a Member, you agree to the following:
- Provide truthful and complete health information during intake and throughout your membership
- Follow the instructions and protocols prescribed by your Provider
- Report any adverse reactions, side effects, or changes in your health promptly
- Use peptide therapies only as directed by your Provider
- Not share, resell, or distribute prescribed medications to any third party
- Maintain a valid and current payment method on file
- Communicate with Project Preservation and your Provider in good faith and with respect
8. Assumption of Risk
You acknowledge and understand that peptide therapy, like all medical interventions, carries inherent risks, including but not limited to: allergic reactions, injection site reactions, hormonal fluctuations, and unknown longterm effects. You voluntarily assume all risks associated with your participation in peptide therapy and related services provided through Project Preservation.
You acknowledge that compounded medications are not FDA-approved and are prepared by a licensed compounding pharmacy pursuant to a valid prescription from your Provider. Compounded medications do not undergo the same regulatory review as commercially manufactured drugs.
9. Limitation of Liability
To the fullest extent permitted by applicable law, Project Preservation, its owners, officers, employees, contractors, and affiliated Providers shall not be liable for any indirect, incidental, consequential, special, or punitive damages arising out of or related to your membership, your use of our services, or any peptide therapy prescribed through our platform.
Project Preservation’s total liability to you for any claim arising from or related to this Agreement shall not exceed the total amount of fees paid by you in the twelve (12) months preceding the claim.
10. Dispute Resolution
Any disputes arising out of or relating to this Agreement shall be resolved through binding arbitration in Dallas County, Texas, in accordance with the rules of the American Arbitration Association. You agree to waive any right to a jury trial or to participate in a class action lawsuit.
This Agreement shall be governed by and construed in accordance with the laws of the State of Texas, without regard to conflict of law principles.
11. Modi!cations
Project Preservation reserves the right to modify this Agreement at any time. Modifications will be communicated to Members via email or through the platform, and continued use of services after notification constitutes acceptance of the modified terms.
12. Entire Agreement
This Agreement, together with the Privacy Policy, Terms and Conditions, Telehealth Consent, and Product Disclaimer published on the Project Preservation website, constitutes the entire agreement between you and Project Preservation regarding your membership and supersedes all prior agreements, representations, and understandings.
